If you’ve put off pregnancy because of work or relationship concerns or because you simply weren’t ready, you may feel like the woman on the 1980s cartoon who laments, “Oh my God, I forgot to have children!” There’s no denying your odds of getting pregnant are now far lower than just a few years ago. After 45, experts say, it’s almost impossible to get pregnant using your own eggs.

At the same time, many 40-plus women do get pregnant, some using fertility treatments and some not. And recent studies have shown there may be benefits to waiting to have children, for both you and your child.

Pros
Probably the greatest advantage of waiting to have children is that you’ve waited to have children. You’ve had time to grow and to see the world; you’re presumably more secure financially and more comfortable in your career. If you’ve been with your partner for a while you will have had the chance to get to know each other in a variety of circumstances, which will provide a solid foundation for raising a family.
There’s some evidence that older mothers, (who are, in general, better educated than young mothers) make wiser parenting decisions. They’re more likely to breastfeed and, according to one recent study, to eat more healthily.

Waiting to have children also has financial benefits. According to one study in the UK, “Women who give themselves more years of experience in the workplace before having children tend to come back to work quicker after their maternity leave. Because they’re more connected to their employer, it’s easier for them to come back part-time if they want to, rather than leave the labour market altogether.”

Susan Heitler, a family and marriage therapist, believes that the best time to have children is in your late 20s to early 30s, but she also sees pluses for couples who wait: “Parents in their 40s are often more focused on their children than younger parents,” she points out. “They’ve had time to travel and to have a broad range of experiences before having children. They have less financial pressure, and more of a ‘been there, done that’ attitude towards hard partying and 60-hour working weeks.”

Nicole, a mother of five and sales director of a large hotel, says that, at 41, she has less energy than she used to, but on the other hand is a wiser and more patient parent. “In your 20s and your 30s, you tend to put too much pressure on yourself as a parent,” she reflects. “You think it will be a disaster if things don’t go just right. If you don’t have time to buy a present before a birthday party, for example, you stress out about it. As an older parent, I find things don’t bother me as much.”

On a practical level, “when you’re a single parent in your 20s and you’re late for work, it can cost you your job, which means you may not be able to pay the rent,” she adds. “Now I’m at a level where I have job security and financial security. I don’t have to worry about things like that, so I’m a lot more relaxed.”

Cons
The biggest downside to putting off pregnancy until your 40s is significant: the longer you wait, the harder it is to get pregnant. This is principally because, as early as 15 years before a woman goes through menopause, the number of her eggs begins to decline and the eggs that are produced are more likely to have chromosomal problems that raise the risk for miscarriage and birth defects.

There’s a big difference in egg viability between the early 40s and the mid to late 40s. “There’s a steep drop in fertility in the 40s,” says Julia Johnson, a professor of obstetrics and gynaecology. “Your odds of getting pregnant at 41 are much better than they are at 43.”

A recent study in the medical journal Fertility and Sterility confirms her point. Researchers found that 40-year-old women treated for infertility had a 25 per cent chance of achieving pregnancy using their own eggs. By age 43 that number dropped to 10 per cent, and by 44 it had plummeted still further to 1.6 per cent. Among women who did get pregnant, the miscarriage rate was 24 per cent for 40-year-olds, 38 per cent for 43-year-olds, and 54 per cent for 44-year-olds.

Fertility expert James Goldfarb says that, in his 30 years on the job, he has never seen a woman get pregnant with her own eggs after age 46. “It’s like buying a lottery ticket,” he says. “Yes, someone wins every once in a while, but you shouldn’t bank on it.” (Note: if you don’t want to get pregnant, you shouldn’t take this to mean that you can stop using contraception at 46. Instead you should wait until you are sure that you have stopped ovulating.)

Using a donor egg boosts the odds of getting pregnant considerably, and according to Dr Goldfarb, that’s how most of the older celebrity mums are doing it, whether they admit it or not. “The fact that they don’t talk about it openly does a real disservice to other women,” he says. “We get at least one patient a month who comes in with the false hope that she can get pregnant using her own eggs.”

Pregnancy complications are another concern. In your 40s you’re more likely to develop problems like high blood pressure and diabetes during pregnancy, as well as placental problems and birth complications. Women over 40 have a higher risk of delivering a low-birth-weight or premature baby. Stillbirth rates are also higher, and studies show that children born to older mothers are themselves at increased risk of type-1 diabetes and high blood pressure.

And don’t forget about your partner; his age can affect his fertility too. Although men are physically capable of fathering children in their 60s and even their 70s, sperm quality deteriorates with age, and there’s a higher rate of genetic defects than there is with the sperm of younger men. In recent years clinical trials have found links between the father’s age and genetically related conditions such as Down’s syndrome and schizophrenia.

All these downsides can be disheartening, but don’t forget that some women do get pregnant well into their 40s (Cherie Blair had her fourth child, Leo, at 45) and many of them have complication-free pregnancies and healthy babies. Plus, while older mothers may be at higher risk for pregnancy complications, the overall risk is low.

One more caveat: while there are financial advantages to waiting to have children, there may be liabilities as well. “If you wait, you’re likely to have to continue working to an older age, for one thing,” says financial planner Marnie Azner. “You’ll still have financial responsibilities at a time when many of your friends are beginning to retire. If you haven’t been putting aside money for retirement up until now, it’s going to get even harder to do so after you have children. Other things become more expensive, too, like life and health insurance. Finally, if you have trouble getting pregnant, the cost of fertility treatments can really add up.”

Your odds of success
Roughly two thirds of women over 40 have fertility problems. Once you pass 40, time is pitiless. You have about a five-per-cent chance of getting pregnant in any single menstrual cycle, according to Sherman Silber, a leading fertility specialist and author of four bestselling fertility books, including How to Get Pregnant. At 40 your chance of conceiving within a year is about 40 to 50 per cent, compared to a woman in her mid-30s, who has a 75-per-cent chance. By age 43, a woman’s chance of pregnancy plummets to one or two per cent.

Why the steep drop-off? Silber says it’s all about the eggs. From the time you reach puberty, with your eggs numbering around 300,000, you’ll lose about 13,000 eggs a year. Over the years this steady drop in egg supply will leave you with about 25,000 eggs by age 37. At 37 comes a precipitous drop in fertility. “By age 43, you’re really at the end of your egg supply,” Silber says, “and your chances of pregnancy are slim.”

Miscarriage rates begin to skyrocket in your 40s as well. From age 40 to 44, the rate of miscarriage is 34 per cent, and it rises to 53 per cent for women 45 and older (compared to 10 per cent at age 20 and 12 per cent at age 30). After age 40, the risk of pregnancy complications, such as high blood pressure and diabetes is twice as high as it is for a woman in her 20s. The odds of genetic problems also jump as you get older: at 40, your chance of having a child with Down’s syndrome is one in 110; at 45 it’s one in 30. Since risks of genetic problems increase in this age group, doctors may recommend diagnostic tests, such as amniocentesis or chorionic villus sampling for women this age.

First-time mothers over 40 also have a 43-per-cent chance of having a caesarean (compared with 14 per cent for first-time mothers in their 20s), although some experts believe this partly due to the fact that older mothers are perceived as “high risk” even if their pregnancy is straightforward. The incidence of low-birthweight and stillborn babies is higher as well.

One very positive fact on your side: there’s no better time in history to try to get pregnant as an older mother, given the range of new fertility treatments now available. While in vitro fertilisation (IVF) success rates begin to fall after age 35, your chance of success using donor eggs remains high, with a live birth rate of around 28 per cent. For women in their 40s whose eggs are donated by a woman in her 20s or 30s, the risk of miscarriage and chromosomal problems is consistent with the age of the egg donor.
